Role of Dexamethasone in Reducing Postoperative Pain Following Cardiac Surgery

This case report describes a 58-year-old man with a giant aortic root aneurysm exceeding 11 cm, severe aortic regurgitation, and reduced left ventricular function who underwent successful surgical repair using a Bentall procedure. Preoperative imaging confirmed no dissection, and emergent surgery was performed due to uncontrolled hypertension and high risk of rupture.

Key intraoperative strategies included axillary artery cannulation before sternotomy, early systemic heparinization, direct coronary ostial cardioplegia, and reimplantation of the coronaries into a mechanical valved conduit. The patient’s postoperative course was uneventful, with marked improvement in cardiac function at 3- and 9-month follow-up.

This case highlights the technical complexity of managing aneurysms over 10 cm, which remain underreported despite the routine use of the Bentall procedure. Alternative cannulation strategies and careful preoperative planning are emphasized to ensure safe outcomes in these high-risk cases.
