This report presents a rare case of a 26-year-old pregnant woman diagnosed with intracranial Ewing sarcoma at 16 weeks gestation. She underwent a successful left occipital craniotomy for tumor resection under general anesthesia. The anesthetic approach emphasized careful cerebral perfusion management, avoidance of teratogenic agents, and tailored hemodynamic support, including transfusion and vasopressors. Postoperative outcomes were favorable for both mother and fetus, with no immediate complications and normal fetal monitoring. Histopathology confirmed Ewing sarcoma, and the patient was referred for further oncology care. The case underscores the need for multidisciplinary coordination and individualized perioperative strategies when managing complex neurosurgical cases in pregnant patients.
